The Rural Health Transformation Program is $50 billion over five years, $10 billion a year, across all 50 states. CMS approved the state plans in December 2025. Now each state decides who gets the money, through its own subgrant rounds and its own deadlines.

Some of those rounds close fast. New York's first round was open for 13 days.
